Surgical Deposit Agreement and Cancellation Policy
Once my pet’s surgery is scheduled at Pomona Valley Veterinary Hospital the reserved time for my pet will no longer be available for other patients, Therefore I agree to submit a non-refundable $100 surgical deposit at the time I request my pet’s surgery to be scheduled.
The surgical deposit will be applied to the surgical procedure. The appointment can be rescheduled no more than 12pm the day before the surgery. If no show or canceled deposit will be forfeited (non refundable)
